ISP (Individual Service Plan):
Done at least once a year with the client and his/her team present, the ISP outlines specific services that the client will receive, including the type of services to be received, number of hours of service per day, days per week, and specific goals that the client is working towards. The ISP also includes pertinent medical, psychological and behavioral information.

MH/DD/AD (Mental Health/Developmental Disabilities/Addictive Disease): Department of Human Resources.

MRWP (Mental Retardation Waiver Program): The MRWP waiver provides financial reimbursement for 11 different services from 24-hour residential services to day program services and respite.

CHSS (Community Habilitation and Support Services): The CHSS waiver "bundles" all of it's components instead of providing the type of "fee-for-service" that the MRWP waiver provides.

ICWP (Independent Care Waiver Services Program): This waiver is limited to adults with long-term, physical disabilities who are or may be at risk of being hospitalized.

Medicaid waiver: Enables eligible people who receive Medicaid to waive certain Medicaid requirements to enable states to cover home and community-based services as an alternative to institutionalization. The Waiver is funded federally and matched by state dollars.

DMA-6 (The Department of Medical Assistance form): This form needs to be filled out and signed by a doctor each year in the Waiver Program participant's birth month to verify and recommend a level of care needed and to maintain eligibility for Medicaid services.

Personal Support Services: One of the services offered under the MRWP waiver. PS services offer a range of in-home services and training to help people diagnosed with a DD continue to live with their families or on their own.

Day Supports: Another one of the services offered under the MRWP waiver. Day Supports allow the consumer to do any one of the following or combination of, day habilitation, supported employment, and community based training.
